Initial Fertility Assessment $250.00
Initial Pediatric Assessment $180.00

15 minute follow up (in person or virtual) $65.00

30 minute follow up (in person or virtual) $105.00

45 minute follow up (in person or virtual) $140.00

60 minute follow up (in person or virtual) $165.00
